These terms apply to the Nature Doctors landing pages, forms and the Baseline Assessment offered on them. By submitting a form or making a payment you agree to them.
The $127 Baseline Assessment includes a short intake questionnaire, one in-clinic testing visit (InBody body composition, fasting glucose and ketones, urinalysis), a written report, and a results call of about 20 minutes with the practitioner who wrote the report. It is a clinic service, not a diagnosis, and it is not a test for Lyme disease or any other specific infection. Any laboratory test recommended afterward is a separate service, quoted to you before it is ordered.
If you go on to book a Lyme-specific first visit with Anand Amin, Registered Osteopathic Manual Practitioner, $100 of your assessment fee is credited against that visit's regular fee of $400. The credit applies once, to that first visit only, is not transferable and has no cash value. It does not apply to laboratory tests or to visits with other practitioners.
After payment, our front desk will phone you within one business day to book your testing visit. Please give at least 24 hours' notice to change or cancel an appointment. If you do not attend and do not give notice, the assessment fee may be forfeited at the clinic's discretion.
If you change your mind before your testing visit, contact us and we will refund the assessment fee in full. Once testing has been done, the fee is non-refundable because the service has been delivered.

These terms are governed by the laws of Manitoba and the federal laws of Canada applicable there.
